Diabetes Prevention Program

You can reduce your risk for type 2 diabetes and gain tools for healthy living.

Did you know that losing a modest amount of weight and increasing your physical activity could help you prevent or delay type 2 diabetes?

IF YOU THINK YOU MAY BE AT RISK, GET STARTED TODAY WITH THESE SIMPLE STEPS.

1) PRE-SCREENING

You can determine your risk by taking the CDC’s Prediabetes Screening Test. You can also download our printable Know Your Risk Score Sheet.

2) CONFIRM DIAGNOSIS

If you scored 9 or higher, schedule a visit to your doctor for one of the following blood screenings to determine your blood sugar (Fasting Plasma Glucose, A1c OR 2-hour Plasma Glucose). Bring the Know Your Risk Score Sheet to go over with your doctor.

3) ENROLL

Call to ​confirm eligibility and enroll in the YMCA’s Diabetes Prevention Program. This is a group based class where you can work with others to learn how to adopt healthy habits to reduce your chances of developing the disease

The YMCA’s Diabetes Prevention Program is part of the Centers for Disease Control and Prevention-led National Diabetes Prevention Program and is nationally supported by the Diabetes Prevention and Control Alliance. The YMCA’s Diabetes Prevention Program teaches strategies for eating healthier, increasing your physical activity and making other changes that will improve your overall health and well-being.
