REGISTER to secure your spot FIRST!
STEP 2:
Go to TN Department of Human Service Link
Information you will need when applying:
(DHS will call asking for this information)
- YMCA Tax ID – 62-0476304
- YMCA of Memphis & the Mid-South (Mailing Address)

STEP 3:
Submit DHS Voucher to YMCA
- To submit your proof of for a DHS Voucher, please fill out the Tuition Adjustment Request Form.
- On the form, select DHS Voucher Uploads as the Requested Adjustment Type.

Request Other Financial Assistance
- Please fill out the Tuition Adjustment Request Form.
- On the form, select the Childcare Fee Assistance Request as the Requested Adjustment Type
- For Early Learning Financial Assistance, you must show proof of application for state childcare payment assistance to receive additional YMCA financial assistance
